ESIC Form 37
The Form 37 of ESI is a statement about the continuation of the employment. The employee should fill out this form if he has been continuously employed for over six months during the year before the form is submitted and he expects to continue to be employed by that employer without interruption. You can continue to contribute to the ESI scheme if you work for more than 78 days in a new company. Form 37 is used to carry over an employee’s existing number when they join a new company. This form needs to be submitted at the regional branch of the State Employee Insurance Corporation.
Form 37 ESI contains information about newly joined organization. You need to fill your details as an insured person, as well as that of the employee’s father’s name, establishment code, last date of ESIC contribution and starting date.
